Booking an Appointment

We would be happy to book appointments for new patients via email. We are currently offering in-office, telehealth, and in-home physical therapy visits. In-home PT visits are limited to mama’s in their fourth trimester or those who have extreme need for in-home services.

Appointment Details

  • All sessions at Enlighten PT are one-on-one with Dr. Casie Danenhauer Humble, DPT or Dr. Lisa Tan, DPT, CWS.

  • Initial Evaluation in Person

    1. 75-90 minutes.

    2. Includes: Full and in-depth evaluation, hands-on treatment, explanation of findings, and a personalized home program prescription.

    • Investment for Los Angeles patients: $350.

    • In-home PT visits are available on a limited basis for new mothers post-partum. Please contact us for pricing and details.

  • Follow- Up Sessions (in person or telehealth)

    • 55 minutes.

    • Visits are often a combination of hands-on manual therapy (when in person), education/ discussion, breathing exercises, gentle stretching, strengthening, and home program progression.

    • Investment for Los Angeles patient: $225.

  • Telehealth/ Virtual Physical Therapy Follow- ups

    • 55 minutes

    • Includes: Full and in-depth history taking and discussion of symptoms, an evaluation based on discussion, explanation of findings, treatment and in-depth education focused on findings/discussion, and a personalized home program prescription and appropriate referrals.

    • Investment: $225

Insurance Details

  • Enlighten Physical Therapy does not contract directly with any insurance, including Medicare/ Medicaid. We are unable to serve these communities at this time.

  • We can provide superbills (in most cases) which our patients may submit to their insurance under “out of network” PT coverage, if that is part of one’s individual benefits. Patients are responsible for knowing their benefits if choosing this option.

  • Patients are responsible for payments at the time of service via credit card, Venmo, cash, or check.

  • Good Faith Estimates: Notice of Good Faith Estimates are provided on all intake forms which will be sent once an evaluation is scheduled & confirmed.
